General Information
North Carolina has a Tobacco Use Quitline at 1-800-QUIT-NOW (1-800-784-8669).
Callers can speak to trained tobacco quitting specialists who, upon
request, can call participants back to check on their quitting progress.
The quitline provides free support and information for all North Carolinians.

Fax Referral Form
Tobacco users ready to quit within
30 days can be referred to the quitline by a signed fax
referral form (English / Spanish) sent by their healthcare provider, so that the tobacco
user does not need to make the first call – the quitline can call
them to initiate the cessation process. The quitline cannot inform
anyone of a caller's participation in quitline services due to confidentiality
issues, however, the caller can request a Certificate
of Enrollment to provide evidence of participation in an evidence-based
cessation support program via the quitline. The certificate is then
sent directly to the caller, not the provider. (Note: To be eligible
for the Certificate, the caller needs to enroll in the callback program). Healthcare providers may receive feedback on patient progress if they are a HIPAA-covered entity.
