Assessing tobacco use with the 5 A's
The NC Quitline will follow up with your patient. Fill out the fax referral form and fax back to the NC Quitline. The quitline then makes two additional contacts at periods that have been proven to be difficult for quitters. For more information on Quitline services click here.

A five-step approach to smoking cessation counseling |
| ASK |
· Ask about tobacco use status at every visit
· Document tobacco use status |
| ADVISE |
· Provide clear, strong, personalized adivce to quit
· Discuss health benefits of quitting |
| ASSESS |
· Assess willingness to quit using tobacco
· Refer to NC quitline for counseling
· If unwilling to quit, provide motivational intervention: The 5 R's - Relevance, risks, rewards, roadblaocks, repetition |
| ASSIST |
· STAR: Set a quit date; tell family/friends; anticipate challenges; remove tobacco products from enviornment
· Help patient obtain social support
· Discuss triggers and coping skills
· Discuss pharmacotherapy
· Provide educational materials |
| ARRANGE |
· Schedule at least four follow ups in person or by phone
· First follow-up one week after quit date
· Congratulate success
· Identify problems / anticipate challenges
